NextGen Office, an industry leading cloud based EHR  has partnered with Provation to provide industry-standard order sets and clinical decision support for hundreds of diagnosis codes, offering our providers suggested medication, labs, radiology and consult orders based on the patient's diagnosis. The Provation Order Set Advisor is intelligent, evidence-based, real-time, and actionable clinical evidence medical content. Provation leverages Wolters Kluwer's UpToDate content to provide these suggestions. To implement the new order sets, we created a new Encounter Header, giving you enhanced access to Patient Scorecard, Encounter Preview, and Encounter Worklist.

Provation Order Sets is an intuitive software with intelligent content gap analysis that works with NextGen Office EHR system to optimize order sets management, while helping improve consistency and efficiency of care, implementation of best practices, regulatory compliance and patient outcomes.

How do I access the order sets within the new encounter?

When you add a diagnosis code to the encounter and the diagnosis code is a part of the order set, an alert shows on the Encounter Header that notifies you of the number of available order sets.

Order sets will contain suggested medications, labs, radiology and consult orders based on the patient's diagnosis.

What options do I have as a provider after the order set is returned?

Providers can choose to:

  • Take action based on recommendations
  • Task suggestions to staff members to complete
  • Add suggestions to a worklist for staff to complete
  • Ignore the order set

Tasking clinical staff quickly and easily from the order set is an efficient and time saving step. Simply check the tasks you want your staff to perform  based on the patient's diagnosis in the encounter. Staff is automatically notified and the provider is in turn notified when the task is completed.

Why are multiple order sets returned for some diagnosis codes?

Several order sets may be suggested for the same diagnosis. Multiple order sets are returned due to the diagnosis at hand, and there are different care recommendations based on factors such as the number of visits with this patient for the diagnosis or the care setting.

Example

For HTN I10, you receive order sets for:

  • Hypertension; Ongoing Management
  • Hypertension; Initial Assessment
  • Hypertensive Emergency

You can choose the appropriate order set depending on the patient.

What is the Encounter Header?

The Encounter Header is a navigation bar within the patient encounter that enables you to quickly view:

  • Encounter Preview and Patient Scorecard data as you navigate through the encounter
  • Plan of Care Recommendations by Provation, a third-party tool that provides recommended care based on the patient's diagnosis and age
  • Encounter Worklist to keep track of encounter tasks

What is the purpose of the Encounter Worklist?

Providers can use the Encounter Worklist as a checklist to keep track of their tasks in the encounter. The worklist is designed to be an encounter-specific task list for the provider and staff to reference to ensure all actions required for this patient's encounter are completed. You can add items to the worklist by:

  • Pushing items from the order sets for staff to complete
  • Type additional tasks
  • As a staff member, what is my responsibility with the worklist?

Providers can push tasks to the worklist. As a member of the clinical team, you should ensure all worklist items are addressed and marked complete.

What is the purpose of the Encounter Preview?

You have increased access to patient demographic information, Encounter Chat, encounter tasks, and a new way to navigate patient encounters.

The new navigation is the most exciting part, as you can now refer to other sections of the note while keeping the current section in context, helping clinicians decrease clicks and increase efficiency.

What is the purpose of the Patient Scorecard?

The Patient Scorecard gives the clinical team an easy way to reference the patient's clinical Quality measures while keeping the screen in context. The clinical team can also refer to it if the patient is leveraging the Patient Portal and can have candid conversations in the office while sending electronic notifications.

In Summary

NextGen Office clinical decision support tools via the new Provation order sets is a major step forward in patient care.  Improving consistency, efficiency of care, implementation of best practices, regulatory compliance and patient outcomes.  A robust clinical decision support system driving successful health outcomes.
