The Decision Support Interventions Certification Criterion The HTI-1 final rule adopts the Decision Support Interventions (DSI) certification criterion (at 170.315(b)(11)). This represents the first substantial revision to the Certification Program’s requirements for clinical decision support related capabilities since 2012. The DSI criterion’s conformance updates reflect an array of contemporary functionalities and configuration requirements as well as an expanded set of information, or “source attributes,” related to evidence-based DSIs and Predictive DSIs. The final rule also establishes a definition for Predictive DSI to mean “technology that supports decision-making based on algorithms or models that derive relationships from training data and then pro
The new Decision Support Intervention (DSI) criteria will bring major changes to current Electronic Health Record (EHR) systems:
Replacement of CDS: The Clinical Decision Support (CDS) criterion will be replaced by the DSI criterion in the Base EHR definition by January 1, 2025.
Two types of DSI: EHR developers must support both evidence-based and predictive DSI, requiring updates to accommodate these new categories.
Source attributes: EHR systems must manage 13 source attributes for evidence-based DSI and 31 for predictive DSI, detailing aspects like development and performance metrics.
Risk management for predictive DSI: EHR developers must implement risk management practices for predictive DSI, including risk analysis, mitigation, and governance controls.
User feedback: EHR systems must allow users to provide electronic feedback on evidence-based DSI.
Transparency and fairness: Updates will be needed to ensure greater transparency and fairness in DSI development, particularly addressing disparities and bias.
USCDI v3 integration: New data elements from USCDI v3 must be integrated into EHR systems, affecting various certification criteria.
Compliance: EHR developers must update their health IT modules to comply with the new DSI criteria by December 31, 2024.
These changes will require significant system updates, impacting data structures, user interfaces, and algorithms to meet the new DSI standards and maintain certification.
Looking for an EHR that is keeping up with the new DSI Standards and regulations?
